Return the completed form by the due date along with your Credit Card information to your school. (We accept cheques payable to Creative Children. No Cash please!)

PART A – REQUIRED SUPPLIES
These are required by your classroom teacher and must be purchased. Part A is ordered as a complete package.
PART B – REQUIRED ITEMS
These are required by your teacher and should be added to your order if you don’t already have them.
PART C – OPTIONAL ITEMS
You decide if your child needs the items. You can add it to your creative pack or supply from home.
IMPORTANT: YOU MUST HAVE ALL ITEMS IN PART B!
Part B are required items by your teacher. This means that the items listed in PART B MUST be included with your school supplies. These items can be purchased by adding them to your creative pack or you can bring them to school from home if you want to re-use existing supplies. Please make sure you have ALL Part A & B items ready for the first week of school.
Additional Ordering Information:
*Verify your information! Orders placed with missing or incorrect information WILL NOT be processed.
* A $20.00 fee will apply to all NSF cheques and credit card charge-backs.
*Missing or damaged pack items: Email us at email@example.com. Your subject line should read “Missing or Damaged Items.” Please include your Order Number, School Name, Student’s Name and Grade.
*Returns: Submit your Return Inquiry by EMAIL to firstname.lastname@example.org Please note: Returns are NOT accepted before September 20, 2019.
