{"id":294,"date":"2018-09-19T13:40:09","date_gmt":"2018-09-19T20:40:09","guid":{"rendered":"https:\/\/elementary.sd42.ca\/harryhooge\/?page_id=294"},"modified":"2026-09-17T14:47:12","modified_gmt":"2026-09-17T21:47:12","slug":"forms","status":"publish","type":"page","link":"https:\/\/elementary.sd42.ca\/harryhooge\/forms\/","title":{"rendered":"Volunteer Application Forms"},"content":{"rendered":"<p>Please read the <a href=\"https:\/\/www2.gov.bc.ca\/assets\/gov\/public-safety-and-emergency-services\/public-safety\/protecting-children\/duty_to_report_pamphlet.pdf\">Let&#8217;s Keep Kids Safe\/Duty to Report Pamphlet<\/a><\/p>\n<p>&nbsp;<\/p>\n<h3><strong>*Only complete packages will be accepted*<\/strong><\/h3>\n<p><strong><u>All Volunteers<\/u><\/strong> \u2013 Please bring the following items to the school office:<\/p>\n<ul>\n<li><a href=\"https:\/\/www.sd42.ca\/assets\/media\/Form-10310.1-Volunteer-Application-and-Consent.pdf\"><strong>SD42 Volunteer Application Form<\/strong><\/a><\/li>\n<li><strong>Criminal Record Check<\/strong> (FREE for school volunteers \u2013 must be requested and picked up in person)<\/li>\n<\/ul>\n<p><strong><u>Volunteer Drivers<\/u><\/strong> \u2013 In addition to the above items, please also provide the following:<\/p>\n<ul>\n<li><a href=\"https:\/\/www.sd42.ca\/assets\/media\/Form-10310.2-Volunteer-Driver-Application.pdf\"><strong>SD42 Volunteer Driver Application Form<\/strong><\/a><\/li>\n<li><strong>Driver\u2019s Abstract<\/strong> (must be obtained within one week of application)<\/li>\n<\/ul>\n<p><em>Please bring a complete Driver Volunteer package into the office along with your:<\/em><\/p>\n<ul>\n<li><strong>Vehicle Insurance Documents<\/strong> (including 2<sup>nd<\/sup> page showing 3<sup>rd<\/sup> party liability)<\/li>\n<li><strong>Valid BC Driver\u2019s Licens<\/strong><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>How do I obtain a Driver\u2019s Abstract?<\/strong><\/p>\n<p>Visit the <a href=\"https:\/\/www.icbc.com\/\">ICBC website<\/a>:<\/p>\n<p>1) Manage my licence &amp; ID<\/p>\n<p>2) Get a copy of my driving record<\/p>\n<p>3) Click on \u201cGet your driving records\u201d<\/p>\n<p>4) Sign in using your BC Services Card account or sign in by verifying your identity with your bank\u2019s Interac Verification Service <em>(this step is new in 2026)<\/em><\/p>\n<p>5) Choose the Personal Driving Record (driver\u2019s abstract) option \u2013 5 Years of claims history<\/p>\n<p>6) enter your personal email address (please do not use the school email address)<\/p>\n<p><em>Please note: if you have a Novice (N) licence, you are not permitted to drive any children other than your own. <\/em><\/p>\n<p>&nbsp;<\/p>\n<p><strong>How do I request a Criminal Record Check?
